TYPES OF SERVICE:

Priority Service   Prices Starting at $95.00*

• Time frame: Two attempts within 3 business days after receipt by the process server.
• Send two sets of the documents to be served VIA OVERNIGHT COURIER with prepayment to: APS #2040-Attn: National Division, 7800 Glenroy Road, Minneapolis, MN 55439-3122
• Please fax a copy of your cover letter and the first page of the summons/subpoena to 800-538-5299 for tracking and quality control purposes.

Rush  Prices Starting at $115.00*

• Time frame: Two attempts within 1 business day after receipt by the process server.
• Send two sets of the documents to be served to the address listed under "Priority" above via OVERNIGHT COURIER.
• Please fax a copy of your cover letter and the first page of the summons/subpoena to 800-538-5299 for tracking and quality control purposes.

Same Day Fax/Email Service  Prices Starting at $190.00** plus $1.00 per page for handling.

• Time frame: Two attempts on the same day the documents are received via fax or email if received prior to 12:00 p.m. CST.
• Please fax your cover letter and one complete set of documents for service to (952) 831-8150 or toll free 1-800-538-5299 OR scan the entire set of documents (preferably multi-page, tiff or pdf format) and email to natl@civilactiongroup.com with the subject line: "Same Day Fax service request attached."
• If requested, you will be contacted within one (1) hour of receipt with an exact cost for the service.
• If CAG is unable to complete service for any reason within the requested time frame you will be contacted immediately.

Surcharges: All fees are for two attempts at one address. If service is made at a second address or at an address other than the address provided, surcharges will be accrued.

• Additional proofs start at $35 each.
• Remote Areas (Unlisted) : may have surcharges. We will advise upon receipt of your order.
• Additional addresses are full charge.
• Saturday Service -- additional $25.00
• Appointment of a Special Process Server: -- additional $20.00
• Fee for advancing funds: Actual Amount + 12%

* Payment can be made by check, money order, Visa or MasterCard. Payment in advance is not required if you have a prearranged account with Civil Action Group.

** Unless you have a prearranged account with Civil Action Group, you must supply credit card information on your cover letter - or instructions for CAG to contact you for that information.

COVER LETTERS:

Register online to create your cover sheet. Go to "Order Online National Service of Process" by clicking on the link at the bottom of this page.

All cover letters should include at least the following:

• Case name, number, and venue
• Attorney's file or reference number (if any)
• (Required for certain priority and rush serves only**) Firm's Federal Express, UPS Account number, OR you will be billed to forward documents to nearest CAG office for service
• Person or business to be served and address for service
• Deadlines for service
• Deadlines for return of proof +(include prepaid airbill OR overnight courier information)
• Under what circumstances and on whom substitute service can be made
• Subpoena Service: Please include a check payable to the witness and/or clerk of court, if applicable for fees. APS/CAG can advance this fee, however, there will be a 12% service and handling surcharge (surcharge min.: $5.00)
• Any other special instructions

**Please note: For Priority and Rush service, if the documents must go through the Minneapolis office, there will be a surcharge to overnight your documents to the nearest available office for service. You can avoid this charge by including your Federal Express, or UPS& account number on the cover letter. CAG will then bill the charges to your account directly.

+ Normally, the Affidavit of Service and/or Affidavit of Due Diligence will be mailed to your office. The standard time for return of service is 10 business days after service attempts are completed. If you need the proof returned more quickly, please include your overnight courier information. If you would like a fax copy of the proof prior to mailing, please be sure to indicate that on your cover letter.

GENERAL INFORMATION:

All fees are for two attempts at one address on one defendant. If Personal Service is required, full fees will be required for each defendant even if they are at the same address. If additional proofs are required for service on one person, there is an additional charge for each additional proof. (See "Surchages").

DO NOT SEND ORIGINAL DOCUMENTS without written instructions and unless required by State Law. (E.g. when Original Summons must be shown to the party being served and then returned with the proof of service).
